The CQC's Maternity and Newborn Safety Investigations (MNSI) program analyzed 92 such incidents, finding that inadequate fetal monitoring occurred in 45 of them (approximately 49%). This included cases where prolonged abnormalities in the baby's heart rate went unnoticed during intermittent monitoring, as well as instances where midwives were distracted by other emergencies and neglected to monitor the baby at recommended intervals
A recent investigation by the Care Quality Commission (CQC) has revealed that inadequate drug administration at regular intervals was a critical factor in many cases where patients suffered severe complications or even died. The CQC's analysis of 92 incidents found that in 45 cases (approximately 49%), the failure to administer medications at the correct intervals contributed to serious health outcomes.
